This licence is only for business users with a registered Unique Entity Number (UEN).
Application Details
Workplaces specified as factories under the Workplace Safety and Health (WSH) Act, must either notify or register their activities with the Ministry of Manpower (MOM) before starting operations.
If your factory engages in high-risk activities, you will have to apply for a Certificate of Registration.
If your factory engages in low-risk activities, you will have to submit a one-time Factory Notification declaring your activities.
If your factory falls under the definition of Major Hazard Installation (MHI), you are required to register the premises as an MHI with MOM, at least 6 months before operations.
From 1 April 2019, construction worksites are not required to be registered with MOM.

- Process Hazard Analysis
Process Hazard Analysis is only applicable for renewable registration ie. For : i. Bulk Storage (of Toxic or Flammable Liquid) Terminal; ii. Petrochemical Plant or Oil Refinery; or iii. Synthetic Polymer or Chemical Plant.

(SHMS) Audit / Internal Review Documents is applicable for all factories except for those under the Factory Notification Scheme. If not submitted during application, documents is to be submitted within 2 month after issue of Certificate of Registration.

MHI assessment form is only applicable for registration of MHI. ie. Any premises processing, manufacturing or bulk storage of one or more dangerous substances specified in the First Schedule of WSH (MHI) Regulations and meets the conditions specified under the Second Schedule of WSH (MHI) Regulations.
